Formerly Osprey Ridge Health and Rehabilitation Center, Creekside invites community to meet staff and tour facility.

Creekside Health and Rehabilitation Center, formerly Osprey Ridge Health and Rehabilitation Center, will begin a new era of patient care with the launch of a new name, new ownership, and a tour of the facility to area residents on Aug. 15 at 6:30 p.m.

The 81-bed skilled nursing facility is now part of Reliant Senior Care, a leading provider of post-acute and long term care, based in Philadelphia. With 22 skilled nursing facilities in the state, Reliant prides itself in providing extraordinary care resulting in exceptional outcomes, in tune with the most recent advances in health care.

Creekside administrator Theresa Slachta led the facility's nursing team for 13 years prior to becoming administrator and assuming the management of business operations.

These resources include a diverse range of programs for long-term, short-term, and post-acute care patients.

In fact, through its new relationship with Reliant Senior Care, Creekside has reestablished the way in which it works with patients, customers, and the community to provide transparency in communication and action.

Through its newly adopted business model, Creekside ensures a smooth transition and seamless coordination of care for patients being discharged from the hospital.

A Registered Nurse Care Transition Coordinator works hand in hand with hospitals to coordinate each patient's care from hospital discharge to facility admission and eventually to discharge. Creekside is leaving no stone unturned when it comes to ensuring that all patients receive the most appropriate and evidence-based care.

To help achieve its mission, Creekside has joined in a partnership with The Wright Center in Scranton. The Center's VP of Patient Care, Safety, and Quality, and one of its physicians, Dr. Jignesh Sheth, will lead the overall care and clinical practice at Creekside.

Dr. Sheth's contributions in the field of geriatric care and patient safety will help Creekside's patients achieve the exceptional outcomes that are the hallmark of Reliant Senior Care.

With live entertainment and refreshments, Creekside will invite patients, families and the community to meet with its compassionate and professional staff and tour the facility on Aug. 15, from 6:30 p.m. until 8:30 p.m.
